Medical Examination letter referred by IRCC having IME or UCI numbers is for others.
Requirements for Canadian Medical Examination
Original Valid Passport/Aadhar Card/Pan Card.
One Photocopy of the ID proof to be brought at the time of medical examination. Please write your correct phone number, email ID and address on the back side.
Letter with work permit in case of worker category.
Letter with IME or UCI numbers when referred by Immigration, Refugees and Citizenship Canada (IRCC).
Any Medical/Surgical record/Prescription/Medicine to be brought at the time of appointment.
In case of children below 5 years vaccination record to be brought.
Spectacles (near and/or distance) if you use.
COVID-19 Vaccination Certificate (with the dates of doses taken).
To come after having Breakfast.
Photo will be clicked at the clinic.
Mode of payment available: Cash/Card/UPI.
Fees (including physical examination, urinalysis, blood tests (HIV, Serum Creatinine and VDRL) and chest x-ray.
Please keep 3-4 hours free for processing of IME.
Women may please avoid Medical Examination during Menstruation.
